Free Immunization Clinic for grades 6-12 (1) 

Dear Middle School & High School Families,

We are pleased to announce that in cooperation with Door County Public Health, Southern Door is once again offering a FREE Immunization Clinic for students in grades 6-12 regardless of insurance status.  The clinics will be provided at all Door County Schools during the 2019-2020 school year; Southern Door's Clinic date will be Monday, October 28, 2019.

Due to a change in State funding, only the annual Influenza (flu) vaccine will be available at the on-site school clinics this year.  Both injectable and nasal mist forms will be available.  Please know that Door County Public Health is still able to provide other required teen vaccines such as TDAP at the Health Dept. office by making an appointment (call 920-746-2234).

Attached you will find the proper paperwork to print, fill out and return to the Middle or High School Office by Tuesday, October 22, 2019.  Forms can also be picked up in the school offices, or mailed to you upon request.  If your family has multiple students in Middle/High School, please complete a form for each child.
